3. Return the school copy to the accounts office and collect receipt

5. Fill the form fully and return to the secretary’s office

Requirements:
1. Diploma in Clinical medicine, Diploma in Public Health Dentistry, and Diploma in Pharmacy, Diploma in Environmental Health Sciences and Diploma in Medical Laboratory and Technology – A minimum of 2 Principal passes at the same sitting in Science subjects of Biology, Chemistry, Physics or Mathematics. – Must not have been out of school for more than three years.
2. Diploma in Anaesthesia – Diploma in Nursing. Or Diploma in Midwifery, or Diploma in Clinical Medicine and Community Health from a recognized Training Institution.
3. Diploma in Health Leadership and Management, 1 year post basic diploma in health related discipline.
