2.Fill out Form K.
3.A registered copy of the constitution of the Community Based Organisation is to be registered.
4.A recommendation from the Sub-county Non-Governmental Organisations Monitoring Committee.
5.A work plan and budget or strategic plan for the Community-Based Organisation.
6.Proof of payment of the prescribed fees.
